The short answer
Save the exact rejection and previous steps. Check whether Instagram offers a further review. If that leads nowhere, additional processing and escalation options can be assessed. A rejection alone does not establish the outcome of your case.
Identify the actual decision
A completed review, a request for missing information and an unavailable appeal button describe different situations. Read the full message and record the date, including any displayed deadline. A login error alone does not establish a rejected appeal.

Prepare a short case history
Include the username, when the restriction began, the wording of the rejection and your earlier attempts. Keep any case references and say whether you still control the original email. A short sequence of dated events is easier to assess than disconnected messages.
Describe the information you have rather than sending passwords, codes or identity documents in an enquiry. If several people manage the profile, nominate one person to maintain the record.
Before trying another review
Check whether the available review relates to the current restriction and what information it asks for. Keep the account history accurate. Repeated templates or contradictory explanations are not a substitute for the facts.
